Medical Forms
- New Medical Form - 2009 - BSA has issued new medical forms effective 2009. Adobe Reader allows you to type data into the form before printing. Parts A & C are required annually. Part B (filled out by your healthcare provider) is required for activities that last longer than 72 hours. The forms are now valid only for one year.
